Transitional-aged youth (with and without developmental disabilities) experience multiple challenges around the time of transfer from paediatric to adult-care. The Good 2 Go Transition Program (The Hospital for Sick Children) is a consultation-style program that supports youth in transition and their parents and families, and the health care and community providers who support them, to ensure a seamless and individualized transition into adult care. Resources to support transition- and transfer-planning will be reviewed in an informative and interactive manner.
